RN Care Manager in Oregon America

More about the job: The RN Care Manager (RCM) is reponsible for managing the nursing care of their assigned residents. The RCM supervises and coordinates with all members of the health care team to develop and maintain a high quality of nursing care for their assigned residents to insure the physical, mental and psychosocial well-being of each resident.

Primary Responsibilities
1. Planning, organizing, directing and evaluating nursing care for each resident assigned
2. Supervision of all members of the health care team involved with the care of their residents and includes direct care staff, ancillary staff (rehabilitation, pharmacy, medical supplies, etc).
3. Serving as liaison between the resident, families, outside services and agencies.
4. Perform and participate in recruitment of qualified staff, hiring, scheduling supervision, performance evaluations, counseling and terminating, to assure adequate and consistant resident care.
5. Additional responsibilities include sharing or occationally assuming full responsibility for treatments, staff development or charge nurse duties.

Education/Licensure/Training:
1. Graduate of an accredited school of nursing.
2. Registered Nurse (RN) with current, unencumbered license to practice in the State
